Can Identical Twins Be Different Genders?
Essentially no. Identical twins come from a single fertilised egg that splits, so both babies start with the same sex chromosomes — and a boy/girl pair on your scan is a fraternal pair, effectively without exception. There are a handful of documented mechanisms that produce sex-discordant twins from one egg, and they are genuine, published, and rare enough to be written up as case reports rather than counted in statistics. None of them is the explanation for an ordinary boy/girl twin pregnancy. If a sonographer tells you one of each, you are having fraternal twins, and you can stop reading here unless you want the biology.
I get why the question is everywhere. Half the internet’s twin content treats “identical boy and girl twins” as a fun maybe, which is how expectant parents end up wondering whether their di-di boy/girl pair might secretly be identical. It is not, and this is the plain version of why.
Why one egg normally means one sex
Sex chromosomes are assigned at fertilisation, before there is anything to split. A single sperm carrying either an X or a Y meets an egg carrying an X, and the resulting zygote is XX or XY from cell one. When that zygote later divides into two embryos — days later, in the case of identical twins — both halves inherit the chromosome set that was already there.
Fraternal twins are a completely different event: two eggs, two sperm, two independent coin flips. That is why roughly half of fraternal pairs are one of each, and why every boy/girl set is fraternal.
There is a second consequence people miss: fraternal twins are always di-di — two placentas, two sacs — because separate conceptions build separate life support. So a boy/girl scan and a di-di scan tell you the same thing from two directions.
The exceptions, named and not sensationalised
Three mechanisms turn up in the literature. All are rare, all involve something going differently in early cell division, and two of the three carry real medical implications for one of the babies — which is the part the “fun fact” articles leave out.
A lost Y chromosome. The best-described route. An XY zygote divides, and in one cell line the Y chromosome is lost — through anaphase lag, where a chromosome fails to make it into a daughter cell, or through mitotic nondisjunction. If that cell line goes on to form one of the twins, that twin is 45,X: chromosomally and physically female, with Turner syndrome, while her twin is a typical XY boy. This is not a novelty; Turner syndrome is a condition with real health implications managed by specialists, and the twinning is incidental to it. The medical literature describes this association in a very small number of reported cases since the 1930s.
45,X/46,XY mosaicism. A related pattern where both twins carry a mix of cell lines in different proportions, occasionally producing discordant external genitalia. Also case-report territory, also a matter for a clinical team rather than a genetics trivia list.
Sesquizygotic (“semi-identical”) twins. The genuinely extraordinary one: an egg fertilised by two sperm, producing twins who share all of their maternal DNA and only part of their paternal DNA — somewhere between identical and fraternal. Two cases have been confirmed and published worldwide, in 2007 and 2019. They can be different sexes. They are also not identical twins, which is the point: the category exists precisely because it is neither.
The honest framing of all three: they are why a careful person says “essentially no” rather than “no”. They are not a reason to think your twins might be a hidden case, and they are not a diagnosis anybody should reach for from a blog post. Anything on this list is found by a clinical team, usually through testing done for another reason.
Sex chromosomes and gender identity are different questions
Worth separating, because the search phrase mixes them. Identical twins share their sex chromosomes, and that does not settle gender identity, which is not chromosomal. Monozygotic twins can and sometimes do differ in gender identity — one of many traits where two people with the same genome diverge, alongside handedness, personality and health. It belongs in the same category as the small genetic and developmental differences that make identical twins two people rather than one person twice.
What your scan can and cannot confirm
- Boy/girl: fraternal, decided. Nothing further to test.
- Two boys or two girls, di-di: probably fraternal, not provably. A minority of identical pairs are di-di, so this is the one case where a cheek-swab zygosity test is the only way to know.
- Same-sex, mo-di or mo-mo (shared placenta): identical. Shared placenta and same sex settle it.

Can identical twins be a boy and a girl?
